When you say you have seen all of these arms without dished lames are you talking about period pieces or modern reproductions? Be careful what you look at for references.

I haven't seen any patterns for a chamfron anywhere. The closest is the one I made for my dog. When I made one, I just placed the shaped plates on some material and traced around them. Then once the fabric was attached to the plates I trimed it down to help reduce the bulk. They have a heavy leather "spring" that helps push them back out into place. A metal spring wire would work better though.

Take some 1/8 in. or so barstock/heavy ga. wire and play around with forming it into buckle shapes. Depends on the style you are going for. Look at period examples and try to figure out how to get that shape.
