I'm not sure that's correct. Until and unless we have doctors on site treating and filling out the reports, it is an incident report, not a medical document so I don't think that it would have to be anonymous. Almost. I've had very good luck with the Duke University Library (it can be searched here). If it's at our library, I'll go get a copy and find a way to get it to you.
