By far, the best i've seen is Talon Armoury. (I may have one of these shields that I can sell you...talk to me tonight at practice) Also, Red Falcon Armoury has nice blanks. If you are at an event they attend I suggest By My Hand as they will cut a shield to your specs on site.

AFAIK,

You may authorize either Sword and Sheild or Spear first. Polearm, Greatsword and 2 Sword are allowed after the initial S&S authorzation.
