There is at least a slight bagginess on the forearm of the Herjolfsnes 63 sleeve, IIRC.

You do kind of have to look for it on the Tres Riches Heures illo -- they seem to be lower household servants of some form. Look at the far left (holding the ewer) and the far right (next to the dog).

We also see servers in what appears to be particolored livery in The Feast of Herod (Giotto, 1320), January in the Tres Riches Heures (c. 1412-1416), Chroniques d’Alexandre (mid-15th century).

Hard to say; I think livery varied based on when it was given out, for what occasion, etc. IIRC, the rest of the chapter goes into more examples.
