The other source of evidence that needs to be examined is accounts of actual combat. I'm not aware of any accounts that indicate a spear piercing a helm. I'm not sure if it applies though. The context seems to indicate that you can decline the fight before it occurs. Not that you can get out during.

You'd think it would be explicitly in the rules.

Just as a side note, grappling, depending on the definition may have occured, but it wasn't a part of the rules.
