You can also find these on English knights during Henry VIII's reign, sometimes with only one shoulder (so the cut of the chest is at an angle), worn over armor by the knights/heavy cavalry. (As opposed to His demilancers &c.)

Smoking - well, this is substantaily a post-period vice. I can't imagine any manner to make it look period. Some people use pipes - but that doesn't solve it and makes others think that they are doing something period - which is wrong. SO, in a sence, it may be better to just use an unobtrusive mod...

Not sure about 16th C. Italy, but Stubbs and Holinshed both refer to the practice of wearing earring/s in 16th C. England. Shakespeare's portrait with the earring is probably the one most folks would be familiar with.
