typicaly, what size head would a "small", "medium", and "large" helmet fit?

Another thing to bear in mind (I learned from annoying experience) is that different folks have different ideas as to how much padding they want. I always did the SCA type thing where leaving room for 1 inch of padding was standard.... but some folks out there want as little as 1/4 inch.... made me too nervous, didn't take the order.
If you were going to make generic sizes though... I'd say
small: 20-22 inches
medium 22-24 inches
large: 24-26 inches
obviously, you'd have some folks fall outside those ranges... and you'd have to know how much padding they wanted to give them the right one.
As a final note, it is probably better to size each helm to each customer when taking orders.

Please note that SCA requirements are for a minimum of 1/2" of closed cell foam.
With a heavy helmet, 1/2" works great. With a lighter helm you'd want more padding.
