Some were attached with leather, with the leather covering the entire backside and being riveted on, with a tab leading to the shoulder. I've got a picture where it looks like there is a horizontal post rivet to it for a loop of leather to go through as well (not sure, couldn't see the backside, just the two rivet heads on the front).

What other ways? Timeperiods?
